Explore who you are and what brings meaning at this point in your life. Join a group of peers to discuss the pros and cons of change and bring clarity to how you define yourself during this stage of life.

Open Position: Part-Time Case Manager/Options Counselor

The Hollywood Senior Center is looking for a compassionate and resourceful part-time Case Manager/Options Counselor (24 hours a week) to join our team.

The Hollywood Senior Center is an independent non-profit community organization providing services and activities for all older adults, from healthy to frail, across a full spectrum of interests and needs.   Hollywood Senior Center is a contractor with Multnomah County Department of County Human Services, Aging and Disability Services Division (ADSD) to provide services for seniors, 60 years of age and older living in Multnomah County that will assist clients in remaining as independent as possible, to delay or prevent out-of-home placement, and support client self-determination.  The Center, working in collaboration with community partners, will provide a broad array of services for seniors including but not limited to: Options Counseling, information and assistance, health and wellness and focal point leadership.

The Case Manager/Options Counselor is responsible for providing Options Counseling services to elders and to family caregivers.  The Options Counselor will provide a holistic assessment of needs, determine eligibility, authorize and coordinate  services, work with elder clients to determine goals, will provide counseling, problem-solving, and advocacy; and will provide a needs reassessment for the client on a regular basis. The Options Counselor could also be trained to lead an additional program related to Options Counseling or other activities as needed.

Compensation and Benefits:  Starting hourly wage of $18.00.  Other benefits include life insurance, paid vacation and sick leave, employee    and employer retirement programs.

Qualifications and skills: 

  • Bachelor’s degree or equivalent in social services or related field
  • Minimum of 2 years experience working as a case manager in social service programs.
  • Familiar with case management practices i.e. interviewing, listening, letting the client set the goals, keeping expectations of the clients reasonable, and encouraging clients to work towards their goals
  • Experience in communicating effectively with and guiding individuals age 60 and over and their care givers.
  • Experience making and monitoring quality referrals and collaborating across agency lines to guide client towards setting and reaching their own goals within the context of available resources and creative problem solving.
  • Demonstrated ability to work effectively, independently, and as part of a cross-disciplinary team.
  • Ability to research, analyze, and organize information about community resources and mobilize and leverage resources to encourage a client to set and reach their own goals.
  • Bilingual written and spoken in a language in addition to English preferred, but not required.

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED OF THE POSITION

Mental:  Must have the ability to function in a positive manner in a demanding work environment, to demonstrate a high degree of flexibility, to respond to priorities and schedules that change frequently, and to meet commitments and deadlines. Must be a strong multi-tasker and able to meet multiple, sometimes conflicting deadlines. Must be detailed oriented and have excellent organizational skills. Must have analytical and decision-making abilities. Must be able utilize a personal computer, read and analyze data in hard copy and on a computer screen. Must be able to deal with distressed and/or demanding customers and employees in an effective manner. Must be good at setting boundaries with clients. Must have the ability to maintain a high level of confidentiality.

Interactive/Communication:  Must be able to effectively communicate verbally, in writing, and by telephone to both individuals and groups. Must be able to listen and assist in clarification of the situation in order to encourage client to problem solve. Must have the ability to communicate with a variety of personality types and levels of the organization as well as with persons outside of the organization. Must be able to communicate well in a variety of ways and situations. Must be able to prepare clear and concise reports. Must have excellent grammar, spelling and writing skills.

Physical: Much of the work is done in client’s homes, in varying conditions of cleanliness and disrepair. Will see hoarding situations and will need to be comfortable and sympathetic.  Must be punctual, maintain a good attendance record, and be able to work flexible hours to meet the availability of participants and customers. Must be able to use basic office equipment such as telephone, copier/fax machine/scanner, and personal computer. Must be able to sit, stand, walk, drive, talk on the telephone and/or use PC for long periods of time. Must possess a valid driver’s license and verification of current auto-insurance, and have full use of automobile during work hours.
